Fees & Payment Information

Birth & Death Certificates       $27.00 Each

We accept all forms of payment, Cash, Check or Credit/Debit Card.

You can obtain death certificates from the county vital statistics office where the event occured.

You can obtain birth records from any vital statistics office regardless of where the birth event occured in Ohio. (Exception: sealed adoption records may only be obtained from the state office of Vital Statistics in Columbus, OH.)

By Mail: Check or Money Order should be made out to:

Please send: Name at Birth, Birth date, Parent’s names (Mother’s maiden name)
Please include a return Name, Address and Daytime Phone Number with all requests.

Over the Phone: We take Credit/Debit for payment to process Birth & Death Certificates.

Birth & Death records prior to 1909 can be found at the Hardin County Probate Court 419-674-2230 located in the Courthouse or you can check with the Hardin County Genealogy Society at 419-674-4088 or on their website:
